Step 2.
- Submit Registration Form along with documents as under:
- At the time of submission, the following documents have to be submitted as attachments.
- Duly filled in registration form.
- A self-attested copy of Birth Certificate.
- A self-attested copy of proof of residence.
- A self-attested copy of SC/ST/OBC Certificate, if applicable.
Step 3.
Scan the QR code and pay Rs. 25 online.
After submission & Payment , you will receive a receipt online with a registration number.
